On most Android versions you can revoke any of these later from the app info screen. An app that refuses to run until you grant something from the no list has told you what it is.

Fake builds: cheap to make, hard to spot

Repackaging is routine work. Take a real build, unpack it, add code, repack, sign with your own key. The icon matches because it was copied. The login screen matches because it is the original screen with your keystrokes duplicated somewhere, or a web view aimed at a domain one character off. Judging a build by how professional it looks is therefore worthless advice: appearance is the cheapest part of the clone.
